In consideration of participation in this selection process, I the undersigned fully agree, understand and acknowledge the following:
- That outdoor activities/games involves physical exertion and other risks, known and unknown and may result in injury.
- Is aware of the possibility of risk of injury, including but not limited to falling, tripping, snake bite, heat stroke, heart attack, hypothermia, insect bites & stings, poisonous plants, bodily injury, disease strains, fractures, eye injury or blindness, death or other ailments that cause serious disability.
- Warrants and acknowledges that my physical condition is excellent and my mental state is sufficiently stable to enable myself to participate safely in outdoor activities and the associated activities.
- Understand and agree that it is my responsibilities to follow all safety rules and procedures explained.
- Understand that any, or all belongings/possessions is left at the sole risk and responsibilities of myself. The organizers and its staff/volunteers assume no responsibility for the lost or damage of belongings/possessions.
In consideration of taking part in the selection programme, the undersigned, on
behalf of himself/herself, he/she successors or assigns, hereby releases and
forever discharge the MNS Pathfinders SIG, MNS, employees, volunteers, land
owners, agents both jointly and severely, from any action, covenants, claims
and demands for damages, cost, loss and injury, however arising, including
negligence of the organizer which may have or may been sustained by the
undersigned in any way relating to or arising out of participating in the
programme or the equipment used including but not limited to the manufacture,
selection, delivery, possession, use or operation of the equipment or the
natural environment. The undersigned desires and agrees to assume any and all
risks involved.
THIS DOCUMENT IS INTENDED TO BE A LEGALLY BINDING CONTRACT RELIEVING MNS, MNS
PATHFINDERS SIG, THEIR EMPLOYEES AND VOLUNTEERS FROM LIABILITY FOR INJURY OR
DEATH TO YOU. IF YOU HAVE ANY DOUBTS CONCERNING ANY ASPECT OF ITS CONTENTS,
CONSULT AN ATTORNEY BEFORE SIGNING IT. THIS RELEASE OF LIABILITY AGREEMENT
COVERS ALL OUTDOOR ACTIVITIES OR EVENTS I PARTICIPATE HEREAFTER.
I hereby agree to the above release and acknowledge I have read each and every term of this waiver.
I understand what each item means and I agree to abide by the terms of this waiver.
